VISION AND MEDICAL INSURANCE
There are two types of health insurance that may provide benefits to help you pay for your eye care services and products. Our practice accepts plans from many providers for both vision and medical.
Vision Care Plans
have benefits that cover routine eye exams as well as eyeglasses or contact lenses. Your vision plan may also cover a basic screening for eye diseases or conditions. They do not cover diagnosis, management, or treatment of eye diseases.
is used for the diagnosis, management, and treatment of eye diseases and injuries. Your doctor will determine if these conditions apply to you, but certain conditions may be considered by case history.
If you have both types of insurance plans, it may be necessary for us to bill some services to your Vision Care Plan and others to your Medical Insurance. Occasionally, this will even result in you scheduling a separate appointment to care for certain conditions. We will use coordination of benefits to minimize your out of pocket expense.
Vision benefits are provided through Vision Service Plan (VSP). We are in-network providers for your plan. Call the office and we can verify your coverage and benefits.
Employees of Adobe have a choice of two Vision Service Plan (VSP) plans. We are providers for VSP so that we can help you maximize your benefits. If you don’t know which plan you have, please call us and we will be happy to look this up for you. In addition, we do have the Optomap instrument for the best evaluation of this inside of your eyes; this is part of your Adobe VSP coverage also.
San Jose State Employees
The VSP Basic plan (Advantage) includes:
• One comprehensive eye exam every calendar year.
• One pair of lenses every other calendar year (or calendar year if your prescription changes) and one frame every other calendar year.
• Contact lenses every other calendar year when contact lenses are provided in lieu of all other lens and frame benefits.
CSU Vision Plan – Premier
All CSU active employees eligible to participate in the CSU VSP Basic Plan are eligible to enroll in the Premier Vision Plan for a small monthly employee cost share. The Premier Vision Plan enables you to get a higher allowance for frames and contacts, fully covered standard progressive lenses, and more discounts.
Eligible employees must enroll through VSP directly by calling (800) 400-4569. The additional cost will be deducted directly from their pay warrant.
If the employee elects the Premier Plan, any dependents they wish to cover must also be enrolled into the Premier Plan coverage. Employees cannot choose to enroll in both the Basic and Premier vision plan coverage at the same time, or split their enrollment leaving any dependents on the Basic Vision Plan.
The VSP Premier plan includes:
• One comprehensive eye exam every calendar year.
• One pair of lenses/frames every calendar year.
• Contact lenses every calendar year when contact lenses are provided in lieu of all other lens and frame benefits.

Stanford Health Care Vision Plan
The vision plan is provided through Vision Service Plan (VSP). This is the VSP Signature plan which provides some very nice benefits that some plans do not cover.
• Optos retinal scanning is covered with a $20 copay. This is an $87 service.
• Non-prescription sunglasses are covered up to the plan limits. Usually, VSP plans do not cover sunglasses.

Tesla employees have several vision plan options. Vision Service Plan (VSP) is the most common insurance. Call the office and we can verify your insurance benefits and coverage elibility.
Cisco Employees have Vision Service Plan (VSP) insurance. We are in-network providers for VSP, and offer the latest technology with our Optomap eye exam scanning. In addition, we have all eyeglass brands from Luxottica, Marchon, Safilo and Kering eyewear. This includes brands like Ray-Ban, Armani, Michael Kors, Ralph Lauren, Valentino, Versace, Prada, Gucci as well as Nike, Lacoste and Nine West. We are a contact lens research site and are a go-to resource for contact lens patients.
If you have the Cisco HSA insurance option, you can use your HSA dollars in our office to cover any fees. Flexible spending account balances may be used for copays, glasses or contact lenses as well.
Pay Pal Employees
PayPal employees have a choice of 2 plans: Core and Enhanced. Both are managed by Vision Service Plan (VSP). Both plans are excellent, with the main difference being that the Enhanced plan provides benefits for 2 pairs of glasses, and higher benefits on contact lenses. Call the office to verify your coverage and benefits.
City of San Jose Employees
Employees of the City of San Jose have the choice between two vision plans, both through Vision Service Plan: VSP Signature, and VSP Choice. We are network providers for both plans. Call the office and we can verify your coverage and benefits.
We are in-network providers for both IBM vision plans: BlueView and EyeMed. Call the office and we can verify your eligibility and coverage.
